PCOS (PMOS), Endometriosis & Fertility: What a Negative Ultrasound Doesn't Tell You

PCOS and endometriosis are two of the most common — and most commonly under-recognized — conditions I see in practice, and they frequently overlap with fertility concerns. Both can take years to diagnose, and both are still widely misunderstood, including by patients who've been told a negative scan means nothing is wrong. It doesn't.

A Quick Update: PCOS Has a New Name

If you've been diagnosed with polycystic ovary syndrome (PCOS), you may have seen that the condition was recently renamed polyendocrine metabolic ovarian syndrome (PMOS). This wasn't a cosmetic change — it followed years of research and patient advocacy pointing out that the old name was misleading. PCOS/PMOS was never really about ovarian cysts; it's a whole-body endocrine and metabolic condition that happens to affect the ovaries, not a gynecological problem with metabolic side effects. The new name better reflects what's actually going on, and hopefully leads to earlier, more complete diagnosis and treatment. You'll likely see both terms used interchangeably for the next few years as the change rolls out.

How These Conditions Are Actually Diagnosed

PMOS (formerly PCOS) is diagnosed using the Rotterdam criteria: you need two out of three of the following — irregular or absent ovulation, clinical or lab signs of elevated androgens (like acne or excess hair growth, or bloodwork showing elevated testosterone), and polycystic-appearing ovaries on ultrasound. That third criterion is the one that trips people up: you do not need a positive ultrasound to be diagnosed. If you have irregular cycles and signs of high androgens, that's enough to meet criteria — a normal-looking ultrasound doesn't rule anything out.

Endometriosis is even more commonly missed by imaging. Ultrasound and even MRI can pick up larger endometriomas (cysts), but they routinely miss smaller superficial lesions and adhesions, which is why laparoscopy has historically been considered the diagnostic gold standard. In practice, this means a huge number of people with real, active endometriosis are told their imaging is "normal" and their pain is dismissed as a result. A negative ultrasound does not rule out endometriosis. Symptom pattern — cyclical pelvic pain, pain with intercourse, pain with bowel movements during your period, heavy bleeding, and fertility difficulty — matters just as much as, if not more than, imaging.

What's Actually Driving These Conditions

Research into both conditions has shifted significantly toward understanding them as whole-body, systemic processes rather than purely reproductive ones.

For PMOS, insulin resistance is now understood to be a central driver in a large proportion of cases — including in people who aren't overweight. Insulin resistance drives excess androgen production, which in turn disrupts ovulation, which is part of why the renaming effort specifically prioritized the word "metabolic." This reframes treatment: addressing insulin sensitivity often does more for cycle regularity and androgen symptoms than treating the ovaries in isolation.

For endometriosis, current research describes it as a chronic inflammatory and estrogen-dependent condition involving genuine immune dysfunction — not just tissue that's "in the wrong place." Recent large-scale reviews point to disrupted innate and adaptive immune responses, oxidative stress, and impaired clearance of endometrial cells that implant outside the uterus, alongside estrogen-driven growth and inflammation at the lesion site. This immune and metabolic angle is exactly why a whole-person approach makes sense clinically, not just philosophically — the drivers genuinely extend beyond the reproductive organs themselves.

Naturopathic Treatment: Targeting the Underlying Metabolic and Hormonal Picture

Because both conditions have real metabolic and inflammatory drivers, naturopathic treatment focuses on identifying and treating those underlying processes, alongside managing day-to-day symptoms.

Inositol is one of the most well-researched options for PMOS. It works primarily as an insulin sensitizer, and the evidence for it is genuinely strong: systematic reviews and meta-analyses of randomized trials show inositol significantly improves the likelihood of regular menstrual cycles compared to placebo, with effects comparable to metformin in several head-to-head studies. Beyond cycle regulation, trials have shown meaningful improvement in hormonal acne and hirsutism (excess hair growth) — in some studies, more than half of participants saw their acne resolve after several months of use — along with reductions in circulating androgens. It's generally very well tolerated, which is part of why it's become such a common first-line nutraceutical for PMOS.

Managing Pain: What the Research Supports

Period pain is one of the most disruptive symptoms for both conditions, and there's genuine evidence behind several non-pharmaceutical approaches.

Diet and exercise both have research support for reducing menstrual pain and inflammation, and are usually the first foundation we build on.

Ginger has been studied specifically for primary dysmenorrhea in multiple randomized controlled trials. Meta-analyses pooling this research show ginger significantly reduces menstrual pain severity compared to placebo, and in head-to-head trials, it performed comparably to common NSAIDs and pain relievers.

NAC (N-acetylcysteine) has a specific and fairly compelling evidence base in endometriosis. Beyond general pain reduction, prospective studies have shown NAC associated with measurable decreases in endometrioma (cyst) size on ultrasound follow-up, along with reduced pain scores and reduced need for pain medication. One cohort study found a striking number of NAC-treated patients were able to cancel a scheduled surgery due to cyst shrinkage or significant pain improvement. It's thought to work by replenishing glutathione and reducing the oxidative stress that contributes to lesion growth — tying back to the immune and inflammatory mechanisms driving the disease itself.

Why a Whole-Person Approach Matters Here

Naturopathic medicine's real advantage with PMOS, endometriosis, and fertility isn't a specific supplement — it's that we don't stop at the reproductive system. We look at the whole person to identify everything that could be contributing: sleep quality, stress load, nutritional deficiencies, gut health, and how all of these interact with mood and energy, which are so often affected alongside the physical symptoms.

Identifying and addressing these contributing factors doesn't just improve quality of life — it can meaningfully support fertility too. Every case is different, and for some people that means working in parallel with a gynecologist or fertility clinic rather than as a replacement for that care — but that root-cause groundwork is often what makes the biggest difference.

What We Actually Look At

Given the metabolic drivers behind PMOS specifically, I always include metabolic markers alongside reproductive hormones, not just the standard hormone panel most people expect.

Metabolic markers:

  • Fasting insulin — this is one I prioritize specifically because it can flag insulin resistance well before fasting glucose or HbA1c become abnormal, which means we can catch and address the metabolic piece of PMOS much earlier than a standard workup would.

  • HbA1c — a longer-term marker of blood sugar regulation, useful alongside fasting insulin for the full metabolic picture.

  • Cholesterol panel — insulin resistance and PMOS are both associated with changes in lipid metabolism, so this rounds out the cardiometabolic picture.

Reproductive and adrenal hormones:

  • Testosterone and DHEA-S — the two main androgens we assess for hyperandrogenism, one of the Rotterdam criteria for PMOS.

  • Estrogen and progesterone — essential for understanding cycle regularity, ovulation, and where in your cycle any imbalance is showing up.

  • Cortisol — I never leave this out. Stress is such a significant factor in hormonal health that looking at the reproductive hormones without considering cortisol only gives us part of the picture; chronic stress can itself disrupt ovulation, worsen insulin resistance, and amplify inflammation relevant to both PMOS and endometriosis.

Other markers:

This is where the whole-person approach really comes in. PMOS and endometriosis don't exist in isolation from your overall health, energy, and vitality — so I make sure we're not missing other contributing factors that can mimic or worsen hormonal symptoms:

  • CBC (complete blood count) — a broad screen that can flag anemia, infection, or other underlying issues that affect energy and overall function.

  • B12 — deficiency is common and can contribute to fatigue, brain fog, and mood changes that are easy to misattribute to hormones alone.

  • Ferritin — your iron stores; low ferritin is a frequent, and frequently missed, cause of fatigue and heavy periods, and is worth checking even when standard iron levels look fine.

  • TSH, and often a full thyroid panel — thyroid dysfunction can closely mimic PMOS symptoms (irregular cycles, weight changes, fatigue) and commonly coexists with it, so it's essential not to assume symptoms are purely ovarian or metabolic in origin.

  • Vitamin D — plays a role in immune regulation and hormonal health, and deficiency is extremely common, especially through Manitoba winters.

Checking these isn't just about ruling things out — it's about making sure we have the complete picture so we can make an informed decision about where to focus treatment.

Testing Options

I know bloodwork and testing can feel like a big step, especially if you've had frustrating experiences with it before. Here's a quick rundown of what's actually available:

  • Standard bloodwork (through LifeLabs or Dynacare): covers most of the markers above — fasting insulin, HbA1c, cholesterol, testosterone, DHEA-S, estrogen, progesterone, and morning cortisol. A good starting point and often the most cost-effective option, though some of these markers may not be covered by provincial health plans — it's best to reach out to your insurance provider directly to understand your specific coverage.

  • DUTCH test (Dried Urine Test for Comprehensive Hormones): a more detailed look at hormone levels and how your body is metabolizing them, including a full daily cortisol curve rather than a single snapshot. There's also an OAT (organic acids test) add-on option, which looks at markers related to mitochondrial function, neurotransmitter metabolism, B-vitamins, and oxidative stress — useful when brain fog or fatigue are part of the picture, since it helps identify contributing factors beyond hormones alone. The kit is shipped directly to your home, and you collect samples over a day or two before sending it to the lab.

  • Salivary hormone testing: measures active, bioavailable hormone levels (rather than total bound hormone, as standard bloodwork often does) at specific points across your cycle, and can also map cortisol patterns throughout the day. The kit is shipped directly to your home as well.

There's no single "right" test — which one makes sense depends on what we're investigating and what you're comfortable starting with.
